PLEASE READ AND SUBMIT YOUR RESPONSE TO ACADEMIC INTEGRITY AGREEMENT - 100 points

PLEASE READ AND SUBMIT "TRUE" for YES (if you agree) and "FALSE" for NO (if you do not agree)  
  • I will abide by the student code of conduct in the ONLINE learning format, just like in face to face classrooms.
  • I will not text, ask other students, search online or use other means to copy answers on the test. 
  • I will continue to use calculators even on the phone.
  • I will only use handouts that are allowed in class. 
  • I will only use the book or notes that are allowed.  
  • I will do the best in maintaining integrity in tests, which means no cheating in any form. 
  • I will have a working camera pointing at me and my table so that I am seen actually taking the test. The camera will remain on during the time of the tests. 
  • I acknowledge that the instructor reserves the right to REMOVE me from the test and disqualify my test, if the CAMERA is not on while I am taking the test.  
  • If I am confused, I will PRIVATELY message the professor via chat in ZOOM without disturbing other students.
  • I am the student who is taking the test and I am not taking any help or support of someone else during the test.
  • Same rules of academic integrity apply to all other assignments which will be done entirely by me and will be a reflection of my TRUE LEARNING.
